We've had several instances where a student has been concluded in a course but no one went in to conclude the student. How is this happening?

Do you by chance have that student connected to any sections that have set end dates? Or, if you're using a SIS and it's connected to your school's Canvas instance, is there something going on there that's syncing over to Canvas that shouldn't be?
This is strange behavior that I haven't really hard of before, so I'm just trying to think of some possible reasons of how this issue could be occuring.

Could there be a feature added (or maybe there is something out there already) to limit not just the file EXTENSION but the file NAME as well? Among all the things that I wish Canvas had is that one! Is it something that is been worked on? We had it in Blackboard (legacy) and I sorely miss and need in Canvas (we just transitioned to it from Bb). Please add this very needed feature!
I have Canvas set to drop the lowest quiz score, however, my students can only barely tell that the grade text is slightly grey when dropped. Can it have a tag like the "missing" and "exempt" ones in the student gradebook?
My Canvas shell has three different sections that all meet at different times throughout the week.
