(Admin) Identify who deleted File or Assignment

We have a case of a large course with multiple Instructors and TA's where an assignment was deleted. The Head Instructor would like to determine who deleted the assignment and provide a training opprotunity. Perhaps the TA didn't realize what they were doing.

We already used the /undelete feature to bring it back, but does Canvas provide such a tool or feature to identify who did it and provide a date/time or when it occurred?

I'm afraid there really isn't any kind of logging that would tell you definitively who deleted a particular assignment. The only way you can really trace that kind of thing down, if you know approximately when the assignment was deleted, is to check the user page views from around that time for all the people who would have had permission to delete it.

Outcomes not working
Our "Outcomes" has not been working for about 6 months. Teachers can't build and attach Outcomes to new quizzes. Canvas said it was a rollover issue. Canvas won't fix it unless we pay them $2000 to "re-train" us for 10 hours. Has anyone had this problem and did it get fixed?
