I'm looking to see if this has happened for anyone else.
A student completed a Classic Quiz and one question was counted wrong that should not have been. In the quiz settings, the instructor entered 9 possible answers for this question. The student entered one of these answers. However, that particular question was marked wrong and only 6 of the 9 possible answers are listed for the student to reference (as seen from the Moderate tab).
The quiz settings appear to be correct. The quiz was taken on an iPhone. I'm not sure what browser the instructor took his screenshots from, but all three of us are seeing the same thing on different devices/browsers.
Upon further review, the instructor realized this had happened for multiple students (but not all). The same quiz in another class had a student who was only graded against 3 of the 9 possible answers.
