Broken link reporting

At Midwestern Baptist Theological Seminary, there is a requirement to identify a local mentor. The webpage with Instructions is here: Week 3: Enlist Your Local Mentor.

Students send a verification link (https://share.hsforms.com/1BODo5ApDSZWw87_dXCuNZgcym8o)  to the mentor, who fills it out and when it's completed, the Grades section shows as complete.

My mentor has completed the online form twice but it's not showing up as completed. I contacted my instructor who recommended I submit this help request to Instructure.

Hi @CoryBrown,

Unfortunately, even though your teacher may have directed you to post here to the global community or to contact Instructure support, this isn't really something either of those areas is able to help with very much.  The form you linked is not directly from Canvas, so if for some reason the form submission is not being received, someone at the seminary would likely need to check with the 3rd party who provides the form.  If your teacher is unsure about the process, there maybe some sort of eLearning or IT support at the seminary you could contact to get further help.
