I have a Canvas shell that's a decade old and am wondering which of the files are actually still being used, and where. Is there any way to go to the Files page and find out this information (such as: a list of pages where the file is referenced)?

Canvas doesn't have anything built=in for this task exactly. There is some good news though, especially if you happen to have some funding available and are an administrator (or can run this past your Canvas admin team). There is a third-party tool called TidyUp, developed by CidiLabs, that is pretty much designed to do exactly what you need. I'd recommend that you check it out.

When you are creating a quiz and/or also reviewing a quiz in Canvas after the class has taken it, the questions aren't numbered so after you get past the first few questions, there is no way to even tell what number you are on. Can someone change that?
