I can't see the item bank in the production environment, only in beta. How do I enable it? Is it a feature that is linked to the choice between classic exams and new exams?

Yes, Item Banks are associated with New Quizzes. Question Banks are associated with Classic Quizzes. If you don't see the Item Banks menu, it would likely be one of two things: 1 - New Quizzes is not turned on for the course or account you're working in. 2 - The item banks menu has been disabled from course navigation. You could visit course settings -> navigation tab to re-enable the item bank menu if this is the case.

When you are creating a quiz and/or also reviewing a quiz in Canvas after the class has taken it, the questions aren't numbered so after you get past the first few questions, there is no way to even tell what number you are on. Can someone change that?
