Does Act As only appear for Admin at the root account?

I do believe you have to assign the "act as" permission at the root account, because technically users all belong to the root account and not subaccounts. You can create a custom role with just the act-as permission and assign admins to that at the root level if you'd like. There is also a feature option to allow admins to act as a user with additional access beyond the admin's (say, a course in another subaccount).
