Are you a student or the teacher in the class? If you're a student and are taking the quiz, I'd highly suggest using an alternate browser if at all possible (Chrome, Firefox, Edge or others should work). Safari has perhaps a little extra security and privacy built in, which can be great overall, but can sometimes cause issues like this with Canvas. Rather than trying to adjust a bunch of Safari settings, I think it's usually easier to just use a different browser for Canvas.
Can you let us know if this helps you work around the issue, or if you still see the same thing in other browsers as well?
