Creating an assignment from a pre-made and fillable google document for students to individually com

I'm trying to upload a document from my google drive that has fillable text boxes.  (i.e. here's a question, answer it in the box below, etc.).  This is an assignment.  I want students to open the assignment, respond, and then submit the assignment.  Each student has his/her own document and responses.

This would require the Google Assignments LTI be on and available in your Canvas LMS. When setting up the assignment as a Google Assignment, it asks if this is a template? That would then create the new copy for each student to use when accessing the assignment and submitting it. If not, then every student will be accessing the same document. That would not be ideal.
