How was the "Canvas_QTI_Package.zip" file created? Was it an export from another Canvas course?
Hello @112610122153523 and welcome to the Community. I'm going to make a BIG assumption here! I'm assuming that you have New Quizzes. This is important because the terminology between Classic Quizzes (the quizzing engine that Canvas has had since its beginning) and New Quizzes (the engine that is in the process of replacing Classic Quiz) is different. Classic Quizzes uses the terminology "Question Banks" and New Quizzes use the terminology "Item Banks". This link, https://community.canvaslms.com/t5/Instructor-Guide/How-do-I-import-a-quiz-from-a-QTI-package-in-New-Quizzes/ta-p/880, should help you import your QTI file into New Quizzes. Please make sure that it is an updated QTI file.
Hello, I am experiencing the same issue and I'm not sure what's going on. I'm still using Classic Quizzes and was able to export an exam, imported it as a QTI file, but it does not show up in the question banks. I'm so stumped
I haven't imported a QB into this specific course, but I did import several QTI files (publisher's test bank) for a different course at the start of the semester with no issues (using the same process).

I am just curiousas to why there is not a field to enter a description for an outcome at the account level? Or at least edit the rating when you attach/link an account level outcome to a rubric. When you create a new criterion for a rubric, you enter a decription for a rating, but for an account level outcome.
