Ungraded icon will not go away

I have a quiz that automatically posts as grade but the ungraded icon is still in my gradebook.  I opened up Speedgrader, wrote comments to the students, and clicked to go to the next student.  When I go out the ungraded icon is still there. Thoughts????

When you view the quiz attempts in Speedgrader, do you see an orange banner at the top of the attempt indicating that there is a question that needs reviewed, like is shown in the screenshot in this article?
