As an Instructor, how can I have students answer questions on a form in Assignments?

I think some additional information from you would be helpful in order to answer your question.  Without knowing exactly what the form would look like, I could imagine a couple possibilities:

• Create a form of some kind as a downloadable Word document or a fillable PDF file.  Students would need to save a copy of the file to their computer, fill out the form offline, and then upload/attach it to an assignment so that you could then grade it via the SpeedGrader.
• If you wanted students to annotate a document that you've created as a form in an online setting (so they don't need to download anything and work offline), you could create an online assignment and use the "Student Annotations" option as described in the section "Select Online Entry Options" of this Guide: How do I create an online assignment? - Instructure Community (canvaslms.com).
• A similar option to the "Student Annotations" option might be to use a Google Cloud or O365 cloud assignment...assuming that your school has set up the LTI (Learning Tools Interoperability) integrations in Canvas for those tools.

• If you are looking to embed an actual form that you've created (such as Google Forms or O365 forms), you should be able to do that by harvesting the embed code from those sites and then adding it into an assignment of your course.  Just keep in mind that the information you are collecting in those forms are stored on Google or O365 and not directly in Canvas...so you'd have to go out to those sites to view the responses.
