How can I require a student to respond to all questions in a quiz? This is a common function of web based forms, online surveys, etc. Is it possible in CANVAS?

I don't see any specific setting in either Classic Quizzes or New Quizzes to require students answer each question. However, if a student did not answer a question and then tried to submit the quiz, both Classic Quizzes and New Quizzes will show a message to the student that some questions were not answered...and that students should go back and answer those questions before they submit the quiz to you.
You can try this yourself by previewing the Classic Quiz or the New Quiz (from a student perspective), and then submit the quiz without answering any of the questions. You'll get a message that tells you questions were unanswered.
