How do you print a report that shows the students who have not completed the benchmark by grade, subject and teacher?

One way I have found to get this kind of report on a benchmark is to go to Admin > Assessments > Benchmarks and select the benchmark name link.
Then go to the Results tab. The graphs may not look like they have all the details, but the data behind them is more detailed than what is displayed. So I go to Export to CSV and get all the data.

The exported data file (classic or expanded) lists all students along with their teachers, trackers, and scores, although not Grade and Subject. (It helps if that information is conveyed in the tracker names.)
You can then sort according to the Score column, and those students without a score are the ones who have not completed the benchmark.

In the Benchmark Results Tab, you can any of the Gray Columns for No Score to display a list by school, teacher, tracker.
It will bring up a report that you can print as a PDF:
