For a teacher who has question banks for different topic areas, how does a teacher make sure that Canvas does not give redundant questions? What to do to avoid pulling the same question multiple times?

I have not noticed this issue in the past, but it happened this semester. I have a bank of 8 questions and the quiz is supposed to pull 3 for each student. Most students are getting 3 different questions, but some students are getting 2 different questions, with the 3rd question being a duplicate of one of the others. How can we avoid that and make sure that 3 different questions are getting pulled from the questions bank?

Canvas doesn't really have a way to screen questions to make sure it is not pulling duplicates from multiple question banks or the same bank linked more than once. As a result we recommend linking a bank to a quiz only once and making sure that there is as little overlap when making a quiz with multiple question banks as possible. I hope this helps.
Among all the things that I wish Canvas had is that one! Is it something that is been worked on? We had it in Blackboard (legacy) and I sorely miss and need in Canvas (we just transitioned to it from Bb). Please add this very needed feature!
I have Canvas set to drop the lowest quiz score, however, my students can only barely tell that the grade text is slightly grey when dropped. Can it have a tag like the "missing" and "exempt" ones in the student gradebook?
My Canvas shell has three different sections that all meet at different times throughout the week.
