Teacher cannot add her name to her course. As Admin I cannot do it either. What should we do?

This is a kludgy solution, but it works. Go into the course that has concluded and go into it's settings, change the term to Default Term, save the change, you will then be able to add the teacher to the course. Once the teacher is in the course, return the course back to the term it belongs in and save the change. You as the Canvas admin can make those changes.
I hope this helps you.
